・ In case of an optional form, please cooperate with us to ensure smooth confirmation of the test certificate by marking the specimen, test method, and other necessary items in the appropriate sections.
・ Medical institution ・ The name and seal of the physician may be regarded as a valid certificate at the discretion of the quarantine officer, since there are circumstances where it is not always possible to obtain the name and seal in each country.

Does the nomi health test result show the date and time of the test ？
Is the specimen method listed in a way that you can return home if you tell them pcr ？?
