I entered Japan on October 16. I have not been vaccinated. It is a US passport.
I showed my insurance card at Palos Verdes Medical Group in Palos Verdes and it was free.
There are a lot of Japanese people there, so when I saw the government form, they said, "Travel to Japan, OK!

I downloaded the government form from the Ministry of Health, Labor and Welfare website, printed it out and brought it with me. That's all.

Procedure
Drop the PDF from the MHLW page and print it out.
Make an appointment for the test on the PVMG web.

Bring your insurance card, ID, and government form on the day of the appointment.
The location is 550 Deep Valley Dr #319, Rolling Hills Estates, CA 90274
Parking at the mall, 4th floor （ Outdoor parking on the roof ） White tent.

There you will take out the ID government form, fill out the checklist, and have a brief Q&A session with the doctor remotely to do a nasal exam.
You will remain in the car. Brief question. I'm in good health ？ I'm vaccinated ？.

test results are in 2-3 hours. Not many PCR tests are done anymore and it was empty, so it didn't take 2 hours.
I'm going to leave for now. I can test in the morning and pick up in the afternoon.

I get a call for pickup and go to the rooftop parking on the 4th floor again.
There you will have easy access to the PVMG office. It is just on the same floor.
You will need to wear a mask to enter the office.
Pick up a piece of paper with the doctor's signed test results and a rubber stamped government form.
Naturally, you don't have to pay because you have an insurance card, it's free.
Then register those two cards in My SOS and you will be blue and ready to go.
It might be a little useful to do Visit Japan as well.

At LAX, show the My SOS screen and the form and they will let you on the plane.
It is natural to wear a mask. The young American who was told to wear a mask got cranky and didn't wear one the whole time.
I wear a mask the whole time I am on the plane. On a different note, ANA had an entertainment set list that was not interesting at all.

When you arrive at Narita, you can connect to the airport Wi-Fi （ The staff will even connect you quickly ） Things pass smoothly when you present your My SOS blue screen.
At immigration, there is no one at the Japanese passport gate anymore. Only the foreigner gate had about 50 people.
Seems like more than half of the whole group was connecting to Southeast Asia.
Passing through immigration was about 2-30 minutes, and customs was over 2-3 minutes after presenting Visit Japann because there was no line.
Both immigration and customs in Japan are friendly and courteous.
But when I left the gate, there was no TV Tokyo crew there.

The travel agency's website says it costs several hundred dollars for inspection, but if you can move around by yourself and have an insurance card, how about this way?

In Japan, everyone wears masks even outdoors. My friend says to be careful, because if you don't wear a mask, you will be treated as a stranger.
He said that even when eating a meal, you must not take off your mask until just before you put the food in your mouth, and then you must put it back on and munch it down.
He told me to think of it as a game. I don't know if this is true or not since I am new to Japan.
At least I often see people drinking through a straw with a mask on, or pumping the sanitizer repeatedly when entering a store.

Japan is a very considerate country. Everyone is polite. Even if you are driving alone or outdoors and there is no one nearby, people wear masks to prevent infection. I even feel nostalgia for the things that the U.S. did right after the pandemic and are still done in Japan. I will try not to be a stranger.

I thought that if you were vaccinated, you could enter the country normally, like before corona.

Excuse me for the sidebar.
I was able to get a free exam at Palos Verdes Medical Group this past July with my Kaiser insurance.
My friend was also able to get a free exam with Kaiser's insurance, so I don't think any insurance company will have a problem.
As Mike's Mike wrote, I received the results about 2 hours after the test. Many of my Japanese friends around me also use this service.

I had a friend visiting from Japan last week, but he had been vaccinated 3 times and returned home without PCR test.
He said that the entry and exit at Narita was very smooth.
Both Americans and Japanese are required to register for My SOS when entering Japan.
If you have not registered, you will be asked to register at the airport when you enter Japan.
It takes about 15 minutes to register, so it is recommended to complete it before leaving the US.
