Tax Man was kind enough to write to you earlier.
Below is a quote from Tax Man's advice.

＞ I have a client who has US citizenship but is not eligible for Social Security
and is currently back in Japan and in a Japanese hospital.
He was an investor and did not work for a company, so he did not pay Social Security or

Tax payment has nothing to do with it. All investors pay taxes.
You are only eligible for Social Security and Medicare if you have the same VISA as the person
whose VISA you are, but he or she has paid Social Security and Medicare properly.

In Japan, people who have not paid their pensions or who have less than 10 years of pension eligibility are ineligible for
benefits.

If you do not have enough credits, you cannot receive the pension.

It's strange that you insist so much
when you can only get the bare minimum of medical care when you get Medicare.

If you go back to Japan for treatment, medical care is better than in Hawaii, and
the cost of medical care is very reasonable, so I simply wonder why you are so concerned about Medicare.

ちょうど今週、眼科に定期検診に行ってきました。
レシートを見るとExamが$115となってます。私は保険で$105引かれています。
もちろん眼鏡を作るなら、さらに検査が追加になります。
眼科に問い合わせてみてはどうでしょうか？
私はアラモアナビルの中にあるEye Care Associates of Hawaii を利用しています。受付には日本人の方もいて日本語で対応してくれる日系の先生もいますよ。

Correction and addition.
#4 reminded me that Cosco and Sam's, as I recall, don't have eye exams per se, just vision exams and tests to make glasses or contact lenses, so that's why they are cheaper. The Exam fee I mentioned before is the cost of the regular eye exam.
Contact Lens Fitting $ 85(I have insurance minus $ 25) Optomap Screeing $ 37(I am not sure what the heck this is. \I'm not sure what this is.
Also, my insurance will cover about ＄ 120 minutes a year to buy contact lenses or glasses.

I don't know this guy, so I can say this: he started the exact same topic almost 10 times, got deleted, and kept starting new topics without thanking the person who gave him long-winded and polite advice in the beginning.
