I have HMSA's Medicare Advantage ™ (PPO ™) insurance here in Hawaii, but usually a family doctor, specialist, or even a dentist will see you first and send you a bill afterwards that takes your insurance coverage into consideration. Lab tests （ X － Ray or blood tests ） also get billed later. As far as I know, the only people who have to pay the full amount out-of-pocket at the time of the medical exam are those who do not have insurance here, as Halo mentioned.

I think it would be better to ask directly to some banks in the U.S. directly. This is safer than relying on unclear word of mouth or information from personal blogs or websites whose credibility is not known. In some cases, if you are suspected of money laundering or other crimes, it could be quite troublesome later on.

A friend of mine uses Dupixent and I asked him about it and he said he gets it prescribed by a dermatologist named Dr. Ryan Sato. He said his friend's insurance is HMSA. He said that without insurance it would cost him $1,000 a bottle, and that he has a friend who has Kiser insurance and gets his prescriptions from other doctors.
