Submission of proof of inspection within 72 hours seems mandatory.
The format should also be properly examined before returning home.

It would be best to check before returning home. You cannot enter the country without a valid inspection certificate.

I see that sometimes certificates are considered invalid.

They can have it issued here.

（ 1 ） GMS Clinic ：
If you give them the format specified by the Japanese government, they will fill in the format directly and create the certificate. This method is relatively easy for you to verify the contents and reduces the possibility of any deficiencies being pointed out. Before taking the test, please present the designated format and request that the test be performed using a test method and specimen collection site that meets the requirements, and that the test results be written directly on the format.

（ 2 ） CMD ：
We cannot ask you to fill in the format designated by our country, and a certificate will be issued in our own format. In addition, it is frequent to find errors in the entry of items, or to be given a certificate in Russian even if you have requested a certificate in English in advance. When you receive the certificate, please make sure that the information is written in English, that the specimen name is "Nasopharyngeal swab" or "Saliva," and that the test date ・ Name ・ Gender ・ Date of birth ・ Passport number ・ Nationality are correct. Please make sure that there are no errors in your nationality. If there is an error, please have it corrected immediately.
In particular, if the test method, specimen name （ specimen collection site ） and date and time of specimen collection do not conform to the format requirements, it is highly likely that the Haneda Airport quarantine will consider the negative certificate "incomplete.
