I have HMSA's Medicare Advantage ™ (PPO ™) insurance here in Hawaii, but usually a family doctor, specialist, or even a dentist will see you first and send you a bill afterwards that takes your insurance coverage into consideration. I think it would be better to ask directly to some banks in the U.S. directly. This is safer than relying on unclear word of mouth or information from personal blogs or websites whose credibility is not known.
