I assisted a person who was planning to bring his dog back to Japan last November, just like you, so here is the information I have for you. First, I think you need to go to the AQS website in Hawaii （ URL below, and prepare the documents according to Checklist 3, after consulting with your veterinarian.

The keys are 1 ． Microchip, 2 ． At least 2 doses of rabies vaccine, 3 ． FAVN antibody test data, 4 ． Complete and submit Form279 （ Vet's signature required ）

I take my dog with me every year. It is difficult every time, but both Hawaii and Japan have import manuals, so if you follow the procedures, you should be fine. The Consulate General of Japan in San Francisco has a detailed list of required documents at the following website. We recommend that you call the Consulate and make an appointment to visit the Consulate after you have all the necessary documents.

Please note that you must use A4/A3 paper when downloading and printing the notification form. （ A4 paper is available on Amazon.com. ）
The only documents you need to bring from Japan are two copies of your family register （ one original and one copy ） but when you visit the consulate, you will need to bring your passport and documents showing that you are legally eligible to stay in the US for identification purposes. Also, if you call the Family Registration Department of the Consulate General, they will be able to help you.
