Hi,
I recently completed the process to change my name, I called the SS office and asked them to tell me how to apply for a name change.
I was told that they now accept all applications by mail, so I sent my passport, green card, Marriage License, Drivers License, and the application form by Certified Mail.
It took 2 to 3 weeks to receive the documents back in the mail, except the application form, and then another 2 weeks to receive my SSN card in the mail.
