I have been in Japan since 12/18 using ANA's LAX-HND flight.

I will need to fill out the government-requested questionnaire on paper that will be handed out on the plane, and
I will need to answer a list of questions on the web created by the Ministry of Health, Labor and Welfare, and if I become friends with someone on LINE
I will receive a QR code, which I can print or show.
When you get off the plane, you will be asked everywhere if you have those two items.
