If you remain in the U.S. longer than permitted, you may be barred from returning home or deported （ from the U.S. ）. Check the Arrival-Departure Record date in the lower right corner of Form I-94 to determine the date your authorized stay expires. It is recommended that you apply for an extension of stay at least 45 days before your authorized stay expires.

You entered the U.S. legally on a nonimmigrant visa
Your nonimmigrant visa status remains valid
You have no criminal offense that would disqualify you from visa status.
I have not violated any conditions of admission
My passport is valid and will remain valid for the duration of my stay
You are not eligible to apply for an extension of stay if you entered the U.S. under one of the following categories

Visa Waiver Program
Crew （ D Nonimmigrant Visa ）
Transiting the United States （ C Nonimmigrant Visa ）
Transiting the United States without visa （ TWOV ）
Fiancée or Dependent of a U.S. Citizen （ K Nonimmigrant Visa ）
Informant on Terrorism or Organized Crime （ and accompanying family ） （ S Nonimmigrant Visa ）
