Why do you need the doctor's signature ？ There was no doctor's signature on the negative certificate. You can get a document from the Ministry of Health and Welfare from the airline company at the airport. You can submit it in Japan and have an antibody test, and if you can prove negative by saliva, you can enter the country. Of course, when you leave LAX, you also need the negative certificate from LAX. The same applies to the time of entry into Japan.

If you bring two certificates, one with the prescribed form specified by the MHLW and the other with the negative certificate issued by the institution where the test was performed, both certificates should contain your name, medical institution information （ Name of the medical institution ・ Seal of the medical institution ・ Name of the physician ・ Signature of the physician ） and the date and time of the test collection can be considered as one negative certificate linking the two certificates. （ Other required information can be included on either certificate ）
