Fremont Cares: There's No Place Like Home

The City of Fremont’s Family Support Services [ https://www.fremont.gov/government/departments/human-services/family-services/family-support-services ] team connected with “Johnathon,” an unsheltered older adult, after he was referred to housing navigation services through Alameda County’s Coordinated Entry System [ https://www.fremont.gov/residents/homeless-response/homeless-services/housing-resource-center-access-point-services ].

At the time, Johnathon was living in his car and experiencing worsening health issues caused by his living conditions.

Recognizing Johnathon's difficult situation, his Family Support Services case manager immediately stepped in to provide critical assistance. His case manager helped coordinate both housing and medical care for Johnathon, while also answering his questions as he applied for housing on his own.

After two years of searching and applying for housing, Johnathon was finally selected for a permanent supportive housing unit. His case manager was able to help Johnathon with the expenses associated with moving into his new home, including the cost of furniture.

For a year now, Johnathon has lived in his safe and stable apartment. Thanks to the help of the City’s Family Support Services, Johnathon has experienced considerable improvement in his health, well-being, and quality of life.
