*Please note that the vaccination coupons currently issued by the city cannot be used for the New Corona Vaccination （ Routine or Optional Vaccination ） after April 1.

・ Mail-in application ： by March 17 （ postmarked ）
・ Apply at the Health Center ： by 17:15 on March 29
（ Apply and receive at the Health Center on March 29 Please consult with the Health Center in advance if you wish to apply at the counter on March 29. ）
・ E-application ： Until March 28
*If you apply over the counter or electronically, applications received by March 21 will be delivered by mail or at the counter, while those received after March 22 will generally be delivered at the counter only.
* For application and delivery at the counter, a copy of the applicant's identification document （ Note ） is required. If you have a proxy, please bring a copy of the proxy's identification document （ Note ） as well.
