医療や介護が必要になっても、住み慣れた地域で自分らしい生活を続けることってできるの？地域の医療と福祉を「知る」きっかけづくりとなることを目的に“医療介護連携フェスティバルin高崎”をオンラインで開催いたします。在宅医療・介護に興味がある方、利用を考えている方、今後のために知っておきたい方、どなたでも見ていただける内容です。
今回は医療と介護の専門職が出演している「人生会議」の寸劇などを通じて、在宅医療・介護に必要な情報を専門職が紹介します。
高崎市医療介護連携相談センター南大類のホームページ上で開催していますので、是非ご覧ください。
※「人生会議」：かけがえのない人生を豊かにしていくことです。今後のあなたの生活への「思い」や知っておいてほしい「好み」について家族等と話し合い、共有していくことです。
