対象の世帯には、市からすでに確認書等を送付しています。確認書等の受付期限は3月29日(金)です(郵送の場合は消印有効)。
確認書の返送がまだ済んでいない場合は、同封の記入例を参照のうえ必要事項を記入し、必要書類がある場合は写し等を添付して返送ください。
直接持参する場合は、保健福祉センターへお越しください。
