高齢福祉行政の推進につきましては、日頃から格別の御協力をいただき厚くお礼申し上げます。
さて、有料老人ホームの設置者は、老人福祉法第２９条第１１項に基づき、有料老人ホーム情報として同法施行規則別表に定める事項を１年に１回以上、都道府県知事等に報告することとされております。
つきましては、標記の報告書を令和７年７月３１日（木）までに、電子申請フォームよりご提出いただきますようお願いいたします。
