     This year, our collective efforts have driven meaningful progress in improving patient outcomes, optimizing care delivery, and managing healthcare resources more effectively. These achievements reflect our shared focus on creating a system that prioritizes value over volume, ensuring better experiences for both patients and providers.

     The holiday season serves as a time to reflect on these accomplishments and to look ahead to opportunities in the new year. *Upcoming Deadlines in BPCI Advanced*

As a reminder, several key deliverables for Model Year 8 are due in December. Please review the following dates and instructions to ensure timely submission:


* *Participant Profile*: Due December 12, 2024, at 12:00 PM EST in the Participant Portal.
* Access the prepopulated template under “Deliverables > PP.”
* Required fields include Waiver and MIPS Improvement Activities on the EI List tab and Quality Measure Set selection at the Clinical Episode level. Empty fields will default to “NO” (EI List tab) or “Administrative Quality Measures Set” (Quality Measures Set tab).
* Submit along with the *Model Deliverable Certification form* in the supporting documents section.

* *Financial Arrangement List (FAL)*: Due December 27, 2024, at 12:00 PM EST in the Participant Portal.
* The FAL template is under “Deliverables > FAL.”
* Lists NPRA Sharing Partners, NPRA Sharing Group Practice Practitioners, and BPCI Advanced Entities, if applicable.
* Submit with the *Model Deliverable Certification form.*

* *Quality Payment Program (QPP) List*: Due December 27, 2024, at 12:00 PM EST in the Participant Portal.
* Access the QPP template under “Deliverables > QPP.”
* Use this to report Eligible Clinicians for QP determinations. Submissions are quarterly.
* Submit with the *Model Deliverable Certification form.*

* *Care Redesign Plan*: Due December 27, 2024, at 12:00 PM EST in the Participant Portal.
* Instructions for the CRP can be found in an email sent separately on 11/07/2024 titled BPCI Advanced – MY8 Care Redesign Plan.

*Additional Information*


* The Model Deliverable Certification form template is located in the “General” section of the Participant Portal’s Document Library and must accompany the respective deliverables.
* Please note there will be no Amended and Restated Participation Agreement for Model Year 8. Participants who do not wish to continue participation should submit a termination notice to BPCIAdvanced@cms.hhs.gov.
