The CMS Coverage to Care (C2C [ https://www.cms.gov/priorities/health-equity/c2c ]) initiative aims to help people understand their health coverage and connect to the primary care and preventive services that are right for them. See the latest news and updates below.

*New C2C Resources*

Learn more about recently released resources to help people understand and access their health coverage:

Cover of updated DOJ reentry resource
* Returning to the Community: Health Care After Incarceration: A Guide for Health Care Reentry [ https://www.cms.gov/files/document/reentry-english.pdf ] – This joint publication by CMS and the U.S. Department of Justice Office of Justice Programs [ https://www.ojp.gov/ ] helps people recently released from incarceration take charge of their health, connect to health services, and find additional resources. It is available in Spanish on the Access Care webpage [ https://www.cms.gov/priorities/health-equity/c2c/access-care ] and more languages are coming soon.
* Tribal Version of the Roadmap to Better Care [ https://www.cms.gov/files/document/better-care-tribal-interactive-v3.pdf ] (now updated!) – This resource has been updated to reflect the latest information for American Indian and Alaskan Native populations and features updated designs specific for Tribal communities. Also included are an updated glossary, additional resource links, and updated contact information. 






Partner Spotlight

C2C relies on trusted community partners like Dawson County Family Partners, to educate their communities about how to get the most out of their health care coverage with C2C.

Last year, Dawson County Family Partners (DCFP) [ https://www.dawsonfamilypartners.org/ ] in Lexington, NE, distributed C2C materials to residents of their community at mobile food pantries, a YMCA Adult Expo, Back-to-School Nights, and the Dawson County Fair. Resources included the Roadmap to Better Care and My Health Coverage at-a-Glance [ https://www.cms.gov/priorities/health-equity/c2c/navigate-your-coverage ]. * *2024 CMS Health Equity Conference* – Thank you to the more than 5,000 attendees who participated in-person in Bethesda, MD and online. CMS leaders and health equity speakers discussed the latest perspectives and research to sustain health equity through action. Stay tuned for the post-conference report and video.
