Request for Applications (RFA) for the ACO Primary Care Flex Model Now Available

The Centers for Medicare & Medicaid Services (CMS) is now accepting applications for the ACO Primary Care Flex Model (ACO PC Flex Model), which aims to grow participation in Accountable Care Organizations and the Medicare Shared Savings Program and increase the number of people with Medicare in an accountable care relationship.

The Request for Applications (RFA) and application portal [ https://app.innovation.cms.gov/PCFlex ] opens today, May 30. Organizations interested in participating must first apply — either as new ACOs or renewing ACOs — to the Shared Savings Program. The deadline to submit an application to the Shared Savings Program [ https://www.cms.gov/medicare/payment/fee-for-service-providers/shared-savings-program-ssp-acos/application-types-timeline ] is June 17, 2024. Applicant ACOs must submit a supplemental ACO PC Flex application questionnaire by *August 1, 2024*. CMS plans to announce applicants selected to participate in October 2024.

The model will launch on January 1, 2025, and run for five years.

The ACO PC Flex Model is a voluntary model that will focus on primary care delivery in the Shared Savings Program. It will test how prospective payments and increased funding for primary care in ACOs impact health outcomes, quality, and costs of care. The flexible payment design will empower participating ACOs and their primary care providers to use more innovative, team-based, person-centered and proactive approaches to care.

To be eligible to participate in the model, an ACO must participate in the Shared Savings Program and be a low revenue ACO (as defined under 42 CFR § 425.20). ACOs that are currently participating in the Shared Savings Program and are interested in also participating in the ACO PC Flex Model must apply to the Shared Savings Program as a Renewal Applicant and begin a new agreement period.

CMS will host a webinar on June 6 to provide an overview of the ACO PC Flex Model’s RFA and application process. The webinar will include a question-and-answer session. Session information and a registration form link are below. Questions can be submitted in advance via the registration form.
