*Collecting ETC Model Participant Information *

If you are or believe you are a Participant in Measurement Year (MY) Six of the ESRD Treatment Choices (ETC) Model (start date July 1, 2023) *please fill out the survey **here* [ https://surveys.cms.gov/jfe/form/SV_2rFEofOBjBMPz7w ]* by 11:59pm ET October 9, 2024.  *New ETC Participants for MY6 were informed of their participation in the ETC Model by mail in May 2023. If you were an ETC Participant in a previous MY and have onboarded to 4i previously, there is no need to submit a separate response for MY6. If you believe you are an ETC Participant but are unsure of the MY during which you were a participant, please fill out the survey.   The purpose of this survey is to gather contact information from new ETC Participants in order for CMS to invite these ETC Participants to access the 4Innovation (4i) platform. 4i is used by ETC Participants to manage their operations in the model. For the purposes of MY6, ETC Participants will be able to access 4i to download their performance reports, called their PPA Report, one month prior to the start of the PPA Period 6 on December 1, 2024. ETC Participants must also have access to 4i to sign a Data Sharing Agreement which will allow them access to beneficiary-level data.

MY6 began on July 1, 2023, and concluded on June 30, 2024. Payment adjustments based on ETC Participant performance during MY6 will be applied during Performance Payment Adjustment (PPA) Period 6, which begins on January 1, 2025. 4i invitations will be distributed to ETC Participants who do not already have an active 4i account in early November 2024.

_Background_

As described in § 512.390(a) in CMS-5527-F, CMS will notify each ETC Participant, in a form and manner determined by CMS, of the ETC Participant's attributed beneficiaries, Modality Performance Score (MPS), and Performance Payment Adjustment (PPA) for a PPA Period no later than one month before the start of the applicable PPA Period.

CMS finalized regulations at §512.390(b) in the CY 2022 ESRD PPS Final Rule stating that the “form and manner” will be via a web-based platform and that ETC Participants will need a Data Sharing Agreement signed in order to receive beneficiary-level data.
