I came from Japan to meet my family with my dog.

Please let me know if anyone has any experience with this.
I came from Japan to see my dog and family at the end of August. I will be going back to Japan soon, but there is one document that is required by the Japanese quarantine 10 days before I go back to Japan, which is called RE \ \ \blur1}.
At the bottom of the document, you need to sign by VET here and endorse by USDA. All other necessary documents are there.
I am having trouble finding a VET who will fill out the form, and when I finally find one, ＄ I am being asked for almost 1000.
Has anyone had an RE form filled out by a VET for returning to Japan, please let me know where and how much it costs.
Also, if anyone knows the market price, please let me know.
