* Not following required risk control plan
* Operating plans not reviewed and approved to ensure safe food handling and storage
* Excessive number of critical violations, including:
* Person in charge unable to demonstrate food safety knowledge
* Food worker cards not available for all employees
* Inadequate handwashing facilities to allow for proper hand washing
* Foods are not being date marked for seven day shelf life
* Improper storage of raw food to prevent cross-contamination
* Room temperature storage of food that must be refrigerated for safety
* Improper hot holding temperatures
* Improper cold holding temperatures
* Foods are not properly reheated before hot holding
* Concentration of sanitizer used for cleaning surfaces too high
* Unapproved changes made to the food facility that affect food safety
