●Whether you are a worker, an entrepreneur, or a full-time homemaker, Japanese in the U.S. are aging as time goes on. Once they retire from the workforce, they will begin to live off their limited assets, which can create financial concerns. In particular, the decline in physical capabilities due to aging will cause anxiety about high medical costs.

● It is not surprising that elderly Japanese living in the U.S. and moving back to Japan cite the high cost of medical care in the U.S. as one of their motivations. But is that true?

●Answer : Yes, it is true. One reality is that 59% of those who declare bankruptcy do so because of medical expenses. 
This is because Japan does not have universal health care as in Japan, but is controlled by private insurance companies. In Japan, the same treatment is administered by the government, so the same price is applied to all treatment procedures. However, the high cost of treatment does not mean that the suffering of illness or injury should be left unattended. Especially for the elderly, whose physical strength is declining, access to medical care can be a serious problem.

●This is where Medicare insurance in the United States comes in.  In the U.S., a public insurance program called Medicare takes care of the health ・ of the elderly after the age of 65. Currently 68.6 million people are receiving it. After Social Security, which accounts for 23% of state spending, Medicare accounts for 15%, and 25%, including 10% for Medicaid, is spent for the nation's health care.

● The American health care system is specialized, they regularly receive the latest training as specialists, are certified as specialized doctors, and can be seen with confidence. At the same time, however, the costs are high and it takes a considerable amount of time to make an appointment and actually be seen.

In addition to Medicare, there is also a Medicaid program to help low-income people (currently 88 million people are receiving Medicaid, about 15 million people in California ), citizens or permanent residents between the ages of 19 and 64 who have low income and little savings). The program is designed to allow people with low incomes and low savings to receive treatment with peace of mind. However, this program is set to be significantly reduced beginning in January 2027, and the state will be forced to cut $ 10 ~ $ 20 billon annually, most of the proposed $ 880 billon cut over the next 10 years ), and the recipients will not be able to work, go to school, or receive treatment. and that recipients must be working, going to school, or volunteering at least 80 hours per month to be eligible for benefits. It is estimated that millions of people will have their benefits terminated.

●By the way, the federal government shut down operations on 10/1/25 because the government and Republicans introduced a bill that would have prevented illegal immigrants from using Medi-Cal, and the House passed it, but Democrats who opposed Medi-Cal compromise the bill in the Senate. The Democrats, who oppose Medi-Cal, said they would oppose a bridge budget in the Senate if the bill was not compromised, so the Administration chose to shut it down.  ( California spends $ 8 . 5 BILLION for illegal immigrant health care )
Traditionally, refugees and refugee claimants, plus illegal immigrants ( over 2.3 million, including some citizens and legal immigrants ) have had regular free access to emergency medical facilities. ( with most costs borne by the federal government and some by the states ), the result of a bargain between the Republicans, who insist on reducing them, and the Democrats, who insist on maintaining the status quo and are trying to increase their electoral vote base.

By the way, when it comes to medical care, the quality of medical treatment ・ the cost of treatment ・ and insurance must be judged as a whole, but since the U.S. does not have universal health care like Japan, one gets the impression that it is cheaper in Japan. Price should not be the top priority over quality when it comes to life-threatening diseases, but that seems to be the reality.

In this respect, as a Japanese senior citizen living in the U.S., I am very grateful for the Medicare system, which allows me to receive inexpensive, high quality treatment. If used properly, medical costs can be lower than in Japan, where 30% of the cost is borne by the patient.
